Dr. Saumyajit Datta will no longer be practicing medicine at South Salem Primary Care – Praxis Health. We want to wish Dr. Datta and his family the best of luck as they transition to the next chapter of his career out of state. You will be missed in the Salem community.
For current patients of Dr. Datta, we have assigned you to one of our other providers to ensure your continued access to care at our clinic. When you call to schedule an appointment, you are welcome to request a change of that assignment. We welcome you to review our team of providers.

Born and raised in India, Sam Datta, MD traveled half the world to arrive in New York in mid-90’s to advance his medical education and training. After finishing his Family Medicine residency, he pursued a fellowship in Geriatrics in Southern California. He then practiced rural medicine in Eastern Oregon managing ER, hospital, clinic and nursing home all at the same time and feels this is where he acquired most of his skills. For about a dozen years, he enjoyed a solo practice in Eastern WA caring for his patients who became part of the bigger family. In 2020, he followed his wife (also a physician in Salem) and became a provider of our medical community.
His approach is to partner with patients in their health and lifestyle choices so that the body, mind and spirit has a chance to heal itself.
Being a family physician allows him to be challenged in a variety of clinical situations, learn from patients and more importantly know about their family and lives. He values the lasting relationships that family medicine offers.
The Datta’s have two grown up kids and a Shih Tzu that fill their lives. He enjoys travelling, photography, classical music and time with friends and family. He volunteers by mentoring the next generation of healers.
Being a new member of the Praxis team, he welcomes patients of all ages to receive personalized and holistic ways of care.
Dr. Datta is recognized as a Fellow of American Academy of Family Medicine (FAAFP). The Degree of Fellow recognizes AAFP members who have distinguished themselves among their colleagues, as well as in their communities, by their service to family medicine, by their advancement of health care to the American people, and by their professional development through medical education and research. Fellows of the AAFP are recognized as champions of family medicine.
